Our website address is: http://crystal-touch.nl. We are a clinic in based in Rotterdam, The Netherlands, who treats patients with complications and residuals after long-standing facial palsy.
What personal data we collect and why we collect it
When visitors leave comments on the site we collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help spam detection.
If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images on the website.
Information filled in by you in our contact forms in only used by Crystal Touch clinic. The information provided is used for the convenience of registering you for the specified event and for personal communication. These forms are powered by Contact Form 7, who do not store this information. The information we receive is stored in our emails without any specified limit, for as long as it is deemed necessary by our employees working with this information. Unless otherwise stated on the form, this information is not forwarded to our marketing communications.
Any health-related information that can be requested from you on our website via our contact forms is only used by Crystal Touch for the purposes of understanding your health condition. This information is not given, sold or transferred to any other websites, platforms or businesses. This information is not analysed by any automatic tools, only by persons working at our clinic. The emails with your health information are stored in our system for as long as it is considered needed by our health specialists. This information can also be used by our specialists to diagnose patterns in the health problems and be used as statistics for our scientific research. In these cases, you, as the source of the information, is kept discrete and anonymous, we do not link any information directly to you and only use it in aggregation with information provided by other visitors.
Our contact form tool – Contact Form 7 – may gather information filled in in the contact form only to be able to send it to us. It does not store any of your personal information on its servers.
If you leave a comment on our site you may opt-in to saving your name, email address and website in cookies. These are for your convenience so that you do not have to fill in your details again when you leave another comment. These cookies will last for one year.
If you have an account and you log in to this site, we will set a temporary cookie to determine if your browser accepts cookies. This cookie contains no personal data and is discarded when you close your browser.
When you log in, we will also set up several cookies to save your login information and your screen display choices. Login cookies last for two days, and screen options cookies last for a year. Who we share your data with
We explicitly do not share your data with anyone. The exception are the addons and plugins that are active on our website who need this data to operate these features on our website. These addons and plugins include MonsterInsight, Google Analytics, MemeberPress, Cookie Notice by dFactory, bbPress and MailChimp. MemberPress uses this information on our website for your convenience, so you can log in, use our member services, and so we can identify you as one of our customers. Most of this information is not used for marketing or remarketing purposes. Only your e-mail, name and the type of goods that were purchased are used for our internal marketing purposes (they are not provided to other third-parties except for MailChimp, where this information is stored. We only use this information if you have explicitly agreed to let us use this information during the registration by ticking the checkbox. If you would like your infromation to be removed, you can click “unsubsribe” in any of the emails you have received from us (powered by MailChimp). Alternative, you can send us a request to remove you from our MailChimp database by sending us an email at email@example.com. Apart from above-mentioned marketing purposes, your information filled in during the registration and stored at MemberPress is only used for you to be able to access our premium content on the website, utilize our forum and login to our website.
In addition, to be able to use our forum, MemberPress is using your registration information, such as your name, email, and other details and forwards it to bbPress. However, MemberPress does not provide bbPress with personal information. Operating on our website, bbPress can collect some infromation from our visitrs. bbPress has their own data-managing policies. We are using MailChimp for our email marketing. The data stored on our MailChimp is not provided to any other websites, platforms or businesses. This information is only used by Crystal Touch for our communication and marketing purposes. Before we are able to collect, store and use this data we always require you to give your explicit agreement for the use of this information for communication and/or marketing purposes. You are given an opportunity to grant your consent to us during your subscription registration via MailChimp or MemberPress, where we explicitly ask you to check the box (or boxes) about whether you agree or not to use your data for our communication and/or marketing purposes. If you would like us to remove your infromation form MailChimp, you can do so by clicking “unsubsribe” in any of the email sent from us to you via MailChimp, as well as by sending your request to do so to firstname.lastname@example.org.
How long we retain your data
If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognise and approve any follow-up comments automatically instead of holding them in a moderation queue.
For users that register on our website (if any), we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.
What rights you have over your data
If you have an account on this site, or have left comments, you can request to receive an exported file of the personal data we hold about you, including any data you have provided to us. You can also request that we erase any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes.
